This is a new area we are developing to help individuals get home quicker with a care team on standby ready and available for them with everything in place rather than remaining in hospital.

This enables us to offer fast track packages for hospital discharge, short term care packages, end of life and emergency care, holiday cover along with Day and Night sits.

Being home is vital when it comes to person centred care and dignity and we are proud to be a team that do this so well with our highly skilled and compassionate care workers.
